Good Faith Exam Policy
At Fort Worth MedSpa, your health and safety are our top priorities. In accordance with state medical regulations and our commitment to providing safe, effective treatments, we require all patients to complete a Good Faith Exam (GFE) prior to receiving medical spa services.
What Is a Good Faith Exam?
A Good Faith Exam is a medical evaluation performed by a licensed healthcare provider (such as a physician, nurse practitioner, or physician assistant). This exam helps us:
- Review your medical history
- Evaluate your current health status
- Determine whether you are a suitable candidate for specific treatments
The GFE establishes a legitimate provider-patient relationship and ensures that any treatments you receive are appropriate and safe for your unique medical profile. We currently perform the GFEs virtually from our office and do everything to get it set up for you!
When Is a Good Faith Exam Required?
- Prior to your first treatment involving prescription-based or medical-grade procedures (e.g., Botox, dermal fillers, laser treatments, etc.)
- Annually thereafter, to update your medical history and ensure ongoing safety
- If there is a significant change in your health or medications at any time
Good Faith Exam Fees
- Initial Exam: $50
- Annual Renewal Exam: $40
Please note that the GFE fee is non-refundable and is separate from the cost of any treatments or services provided.
How to Schedule
Your Good Faith Exam is scheduled at the same time as your treatment or as a standalone appointment. To ensure compliance, the exam must be completed before treatment can be administered.
